I confirm that I have held my active CKBD or CKD certification for a minimum of six (6) continuous, uninterrupted years.

I understand the cost of this application is $550 for current CKBDs and $750 for current CKDs. (Note: CKD candidates must also complete a bath exam and bath portfolio review).
